· Prenatal Care Referral

Care Coordination is responsible for linking women in Palm Beach County to prenatal care. It is our objective to assist women with choosing a Prenatal Care provider and obtaining a payor source for their prenatal care as well as hospital charges. It is our goal to ensure that all women in Palm Beach County enter prenatal care.

Each Care Coordinator is a trained professional who conducts psycho-social assessments as well as financial assessments to identify client's needs. Our Care Coordination team is one of the first to be trained in Dr. Brazelton's Touchpoints curriculum in Palm Beach County.

Care Coordination provides ongoing support, education and referrals to the families in which we serve. We collaborate within the Healthy Beginnings System to provide a continuum of care for those requiring more intensive services. These services may include: WIC, Breastfeeding Classes, Childbirth Classes, Parenting Classes, Healthy Start/Healthy Families Nurses, Education/Support, Smoking Cessation, Women's Health Initiative, Mental Health Counseling Services.

In an effort to make our services accessible to the community, we have Care Coordination services available at the Belle Glade Health Department, Delray Beach Health Department and Lantana Health Department. In addition, healthy mothers/healthy babies has offices in West Palm Beach, Lake Worth and Delray Beach.

We continue to work collaboratively with community agencies to provide quality services to the women and families in Palm Beach County.

· Preconception Classes
These informational classes are offered by Healthy Mothers/Healthy Babies for those women considering pregnancy. Women learn the steps to take to be as healthy as possible before pregnancy.

Childbirth Classes

Our childbirth classes (FOCEP) teach infant and maternity topics. These classes are taught at local high schools.

· Maternal Health Care Navigator Program
Staff work with pregnant women who are having trouble obtaining insurance to cover their pregnancy. This included providing transportation, gathering documentation, filling out forms and other assistance as required.

·Prenatal Outreach
Targets at-risk and hard-to-reach women and encourages them to obtain regularly scheduled health care and dental appointments, complete their prenatal, well-baby and immunization medical visits. The program provides referrals to other social services when needed, and helps clients in accessing these services.

· Momcare
The Momcare program staff contact all pregnant women who have applied for Medicaid and explain their health care options including private doctors, midwives and clinics. Educational materials are sent to women enrolled in the program.

· Hospital Liaison Program
Staff visit mothers, who have just delivered babies, at each of the county's hospitals with maternity services. They help these new mothers enroll for services, such as home visits by nurses, receiving immunization reminders and information on healthy child development.

· Family Assistance Pantry
Each of the seven healthy mothers/healthy babies' offices has a pantry to provide diapers, wipes, baby clothes and other baby items (when available) to families in need. If you have any items you would like to donate to our pantries, please email czambrano@hmhbpbc.org.

· Fatherhood Support
The Responsible Fatherhood Initiative educates dads on the importance of a father's involvement in the life of a child. The groups address issues such as discipline, parenting skills, play, child support, visitation and DNA testing.

· Education on Parenting
Raising Great Kids offers classes to parents of children ages 0-3 and 3-5. These topics include bonding, development, language skills, nutrition, discipline and safety. Lunch and Learn provides classes in preconception, pregnancy, parenting and special classes just for dads at local corporations during lunchtime. · Girl Talk
Classes for girls between the age of 11-19 to teach them life skills, goal setting, self-esteem and birth control. This program ultimately will help in the prevention of teenage pregnancy.

· Guy Talk
Consists of one-hour after school life skills management training boys, ages 9-12 at various subsidized housing development sites. Topics covered are: Anger Management, Conflict Resolution, Peer Pressure, Puberty.

· Conflict Resolution Classes
Elementary school children learn how to effectively handle conflict, peer pressure, anger and other challenging social interactions at school.

· Teen Education
Former teen parents educate middle and high school students about the consequences of teen pregnancy. Birth control, sexually transmitted diseases and male responsibility are also discussed.

· Babysitting 101
Teaches adolescents the basics of babysitting. Topics include safety, child development, play and nutrition.
