Community Outreach is a crucial component in reaching women and teens in Palm Beach County who are at-risk for poor birth outcomes. Healthy Mothers, Healthy Babies system of care operates as a comprehensive process for pregnant women, beginning with outreach in communities where women deliver babies with low birth weight and prematurity which often result in infant mortality. Our team of outreach professionals target at-risk and hard to reach women and teens and educate them on services available in their community that will provide the assistance they need to deliver a healthy baby. HMHB is able to connect women and families to additional services within the Healthy Beginnings system of care through the Children’s Services Council of Palm Beach County.
Healthy Mothers, Healthy Babies Outreach staff also participates in community events throughout the month including health fairs, back to school events and church fairs.
To further our efforts in the community, HMHB takes our message on the road aboard the Maternity Care-a-Van. This mobile outreach allows Healthy Mothers, Healthy Babies to deliver what moms need where and when they need it. Our team collaborates with partners in the community such as grocery stores, medical centers, schools and community groups, and provides a predictable presence of the van in areas where our clients reside and frequent on a regular basis.
