Healthy Mothers, Healthy Babies is the only MomCare provider in Palm Beach County and was developed as a partnership between the Florida Association of Healthy Start Coalitions, Florida Department of Health, Florida Agency for Healthcare Administration and Florida Department of Children and Families. Each county within the state of Florida operates a MomCare program through the Healthy Start Coalition. MomCare is a Medicaid-funded program authorized by a special waiver from the federal government called the Sixth Omnibus Budget Reconciliation Act (SOBRA).
MomCare is available to women who meet specific requirements by the state who are then assigned to HMHB by the Florida Department of Health. Our team of MomCare Advisors provides information, education, referral and assistance for women covered by SOBRA Medicaid through a one-to-one care coordination service.
MomCare Advisors will help you by:
Assisting with the selection of a health care provider for you and your unborn baby
Connecting you with additional community resources that may be helpful
Providing links to childbirth, breastfeeding and parenting classes, as well as smoking cessation
Assisting with enrollment in the Women, Infant and Children’s Nutrition Program (WIC)
Ensuring that you are seeing your doctor regularly
