If you are pregnant and eligible for Medicaid, then this program is for you!
MomCare is a partnership of the Florida - Healthy Start Coalitions, Department of Health, Agency for Health Care Administration, and the U.S. Centers for Medicare and Medicaid Services.
The Purpose of MomCare:
- Provider Choice Counseling:
- To help pregnant women to understand their choices and select a primary medicare provider.
- To assist pregnant women in receiving early prenatal care.
- Case Management:
- To identify at-risk women through the Healthy Start screening process.
- To assist at-risk women to obtain needed services.
The Goals of MomCare:
- Increase Healthy Start screening rates
- Increase participation in WIC
- Assign primary care provider within 30 days
- Identify client needs
- Increase family planning services
- Ensure health insurance for newborn
MomCare Components:
Eligibility: Clients must be pregnant and eligible for Medicaid under income criteria for presumptive eligibility or SOBRA (185% poverty).
Initial Contact: Early contact with pregnant women encourages and assists them to obtain prenatal care as soon as possible, complete the Healthy Start screen and access other needed services.
Follow-Up: Ongoing care coordination and communication through pregnancy assures that women receive needed services.
