Care Coordination is responsible for linking women in Palm Beach County to prenatal care. It is our objective to assist women with choosing a Prenatal Care provider and obtaining a payor source for their prenatal care as well as hospital charges. It is our goal to ensure that all women in Palm Beach County enter prenatal care.

Each Care Coordinator is a trained professional who conducts psycho-social assessments as well as financial assessments to identify client's needs. Our Care Coordination team is one of the first to be trained in Dr. Brazelton's Touchpoints curriculum in Palm Beach County.

Care Coordination provides ongoing support, education and referrals to the families in which we serve. We collaborate within the Healthy Beginnings System to provide a continuum of care for those requiring more intensive services. These services may include: WIC, Breastfeeding Classes, Childbirth Classes, Parenting Classes, Healthy Start/Healthy Families Nurses, Education/Support, Smoking Cessation, Women's Health Initiative, Mental Health Counseling Services.

In an effort to make our services accessible to the community, we have Care Coordination services available at the Belle Glade Health Department, Delray Beach Health Department and Lantana Health Department. In addition, healthy mothers/healthy babies has offices in West Palm Beach, Lake Worth and Delray Beach.

We continue to work collaboratively with community agencies to provide quality services to the women and families in Palm Beach County.
